The goal of our research study is to investigate how an individual’s dietary and lifestyle choices affect wellness, including hormonal/sexual functioning throughout the middle and later years. Your responses to this survey are entirely anonymous and will only take about 25 minutes to complete.

Wading through the maze that is "survey monkey", I eventually gave up. I had the feeling that the site was taking names to sell lists.
