CALL AHEAD! Actually, call before you even go to the island! and then call again before driving to the place to confirm they'll be open and they are still in business! We relearned this lesson about 5 times. Driving in PR is insanity so get the extra insurance. Finding places is nearly impossibly. Road signs are poor and most addresses aren't in navigation systems. You really must speak spanish to have a good time here.
There are 4 Fresh Marts. http://www.freshmartpr.com they are all really hard to find!!! And rare if anyone speaks english. I believe all have a Veg Cafe. (the two we went to did) Mind the times, if you get there between breakfast and lunch don't expect to get served.
Also, the definition of Vegan in PR is a little new and vegetarian is a little loose. Thank god for our "vegan passport" even with it we still had trouble!!!

Veggie Garden is now open on Saturday Evenings form 6 pm - 10 pm. They just opened a wonderful Garden Patio Cafe out back! Please add this info if possible!

Cafe Berlin was the best and most comprehensive vegetarian restaurant in Old San Juan. I guess if you are hung up on being in a restaurant with any meat on the menu, then you might take offense, but for me it did not.
It meant that I and the other 5 guys in our party could share a meal in a great restaurant with great service and a great selection of wonderful food.
They even had soymilk alternatives for those vegans in the crowd.
As well it was only $120 for six people with cocktails and desert.
This is a must do restaurant in Old San Juan.
